The guard shall be such that it does not offer an accident hazard in itself.    a.  On or about 2/21/19 the employer did not ensure that guards were effectively secured to the Viking packaging machine, which exposed employees to amputation hazards. The machine came with guards that had an interlock, but the interlock was bypassed by the contacts being taped together.

(c) Machine guarding was not provided to protect employees from ingoing nip points and rotating parts hazards. The "Puppy Pad machine 3" was set up with interlocks but the employer did not insure that they were functional during operations. This exposed the employees to caught in hazards while clearing the machine housing of overrun product.    (d) Machine guarding was not provided to protect employees from ingoing nip points and rotating parts hazards. The "Puppy Pad machine 4" was set up with interlocks but the employer did not insure that they were functional during operations. This exposed the employees to caught in hazards while clearing the machine housing of overrun product.      The Gardner Pet Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1):, which was contained in OSHA inspection number 1296692, citation number 1 item number 1 and was affirmed as a final order on 3/19/18, with respect to a workplace located at 2845 Wingate Street West Bend, Wisconsin 53095.
