On 2/13/19, at the Blairsville facility, the employer did not conduct a periodic inspection of the Lockout program to ensure deficiencies were corrected. Deficiencies found during the inspection included: identification of energies and the magnitude of those energies, identification of the wrong machine and the sizing of the machine.

On 2/13/19, at Line 4 in production, employees were exposed to the point of operation of the wire (coming off the spool) during threading of the wire into the cone feeder, at the Flyer Payoff, and after welding the wire and holding the wire to be fed into the spool. On 1/22/19 an employee received an amputation after he reached to grab excess wire that had fallen on to the wire coming off a spool and being fed into the machine.
