29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    (a) Future Foam Inc., at 11475 E. 40th Ave Denver, CO: On and preceding 2/20/19, the employer did not ensure that signs were posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent.  Employees performed functions on the east side of the warehouse, including using a conveyor to move material, including polyurethane foam.  This area was also used to store large pieces of polyurethane foam.  Exit route signs were not visible in this area.  The direction to the nearest exit was not apparent.  This condition exposed employees to a fire hazard.

29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  Future Foam Inc., at 11475 E. 40th Ave Denver, CO: On and preceding 2/20/19, the employer did not ensure point(s) of operation of machinery were guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).  Employees operated vertical band saws to cut pieces of polyurethane foam.  The Edge-Sweets model E2 vertical band saw blade was not guarded in a manner that prevented the operator's hand from being in the path of the saw blade.  This condition exposed the operator to an amputation hazard. The drill press was not guarded in a manner that prevented the operator's hand from being in the path of the drill bit.  This condition exposed the operator to an amputation hazard.
