The floor drain in the Recycle Room was left with the drain cover off leaving an uncovered hole in the floor.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees were exposed to amputation hazards.  The panel cover for the Palletizer Discharge was not secured. The light curtain did not guard this entry point to the Palletizer Discharge as this panel was shorter and did not break the plane of the light curtain.  This exposed the employees to amputation and crushing hazards from moving parts, and pinch points from the Palletizer Discharge.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    Employees were exposed to amputation hazards.  The guard for the Palletizer Discharge was hinged and not secured. The light curtain did not guard this entry point to the Palletizer Discharge as this door was shorter and did not break the plane of the light curtain.  This exposed the employees to amputation hazards from ingoing nip points from chain and sprockets from the conveyor.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    Employees were exposed to electrical hazards.  The on and off switch box for the support pole mounted fan in Aisle 3 of the East Side of the Warehouse was exposed to physical damage from forklifts and pallets. There were no guards to protect it from damage.

The conduit was not connected to the on and off switch box for the support pole mounted fan in Aisle 3 of the East Side of the Warehouse.  The conduit had been knocked loose from the on and off switch for the fan.
