29 CFR 1910.124(h)(3): Employees working with liquids that could burn, irritate, or otherwise harm their skin were not provided with appropriate first-aid supplies that were located near dipping or coating operations:  On February 12, 2019, in the Plating Department, the employer did not provide appropriate first aid supplies near the electroplating operations and the plating employees were exposed to contact with corrosive plating solution.

In March 2019, in the Plating Department, the employer did not determine the 8-hour TWA exposure of the electroplating workers that were exposed to hexavalent chromium while electroplating parts.

29 CFR 1910.1026(h)(1): Where a hazard is present or is likely to be present from skin or eye contact with chromium (VI), the employer did not provide appropriate personal protective clothing and equipment and ensure that employees use such clothing and equipment.  On or about April 18, 2019, in the Plating Department, employees were exposed to hexavalent chromium while electroplating parts and the employer did not ensure employees use:  a) aprons;  b) face shields; and  c) chemical protective boots or boot covers.

29 CFR 1910.1026(i)(3)(ii): The employer did not ensure that employees who have skin contact with chromium (VI) wash their hands and faces at the end of the work shift and prior to eating, drinking, smoking, chewing tobacco or gum, applying cosmetics, or using the toilet.  On or about April 18, 2019, in the Plating Department, the the electroplating workers were exposed to hexavalent chromium while electroplating parts and the employer did not ensure employees wash their hands and faces:  a) at the end of the work shift; and   b) prior to eating, drinking, and smoking.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   Department of Army, Rock Island Arsenal, Rock Island, IL: On or about February 22, 2019, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2019.  On or about February 12, 2019, a recordable injury occurred when an employee received prescription medication due to chemical contact which occurred while the employee was troubleshooting a pump in the Plating Department.
