29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used, under the supervision of a qualified person: A) Second floor west side of old recovery boiler #4, on or about February 21, 2019 and or times prior to, a yoyo (self-retracting life line) was attached to a horizontal wire rope that had an eye in one end made from a knot in the wire rope, also, there was a chain hoist attached to it. The fall protection system was not designed, installed and used under the supervision of a qualified person, exposing employees to a fall hazard.
