29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.      On the east side of the construction site, employees engaged in metal installation activities were not wearing hardhats while working below other workers with tools exposing them to struck-by hazards of falling tools and materials.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.        On the east side of the construction site, employees installing sheet metal on a metal building were working from an elevated work platform  that was not an approved work platform by the manufacturer of the JCB 930 forklift for capacity, construction use, and fall protection.   Employees were exposed to fall hazards of more than 16 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:(a)(LOCATION)(TYPES OF FORKLIFTS USED)(DESCRIPTION OF PLATFORM)(DESCRIBE HAZARD(S) WHERE NECESSARY)      On the east side of the construction site, employees installing sheet metal on a metal building were working from an elevated work platform  that was not attached to the JCB 930 forklift exposing the employees to an unprotected fall hazard of more than 16 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      On the east side of the construction site, an employee installing sheet metal from the roof of  a metal building without fall protection was exposed to a fall hazard of approximately 16 feet.
