29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a. On or about February 26, 2019, the employee working on the ground underneath another employee was not wearing a hard hat and was being exposed to head injuries.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about February 26, 2019, the employee operating a nail gun was not wearing eye protection and was being exposed to eye injuries from flying debris.  b. On or about February 26, 2019, the employee operating a circular saw was not wearing eye protection and was being exposed to flying debris.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On or about February 26, 2019, the employee sitting on the roof to replace rotten wood was not protected from falling 9 feet to the ground by the use of a fall protection system.
