29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a. On front of home, at a worksite located at 3018 Myra Street, Jacksonville, Florida 32205:  On or about August 31, 2022, the employer exposed four employees to fall hazards, in that, the 20-foot Little Giant extension ladder was not extended 3 feet above the landing.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. At a worksite located at 3018 Myra Street, Jacksonville, FL 32205: On or about August 31, 2022, the employer exposed five employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down roof shingles.  Synot Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1380782, citation number 01, item number 002 and was affirmed as a final order on April 15, 2019, with respect to a workplace located at 3776 Beach Blvd, Jacksonville, FL 32207.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 5:12 pitched roof, at a worksite located at 3018 Myra Street, Jacksonville, Florida 32205:  On or about August 31, 2022, the employer exposed four employees to fall hazards, in that, fall protection was not utilized by employees while nailing down roof shingles.  The employees were exposed to the hazard of falls ranging from 9 to 19-foot to the ground.  Synot Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1380782, citation number 01, item number 002  and was affirmed as a final order on April 15, 2019, with respect to a workplace located at 3776 Beach Blvd, Jacksonville, Florida 32207.
