29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     On or about February 27, 2019, Blackhawk Roofing, LLC had employees exposed to falls greater than 11 feet without fall protection, on a low slope roof in Springfield, Illinois.     BLACKHAWK ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1049637, CITATION NUMBER 1, ITEM NUMBER 2A, AND WAS AFFIRMED AS FINAL ORDER ON MAY 28, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 1101 E MONROE STREET, SPRINGFIELD, ILLINOIS 62703.     BLACKHAWK ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1274715, CITATION NUMBER 2, ITEM NUMBER 1a, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 5, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 2126 S MACARTHUR BLVD, SPRINGFIELD, IL 62704.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      On or about February 27, 2019, Blackhawk Roofing, LLC had employees exposed to falls greater than 11 feet, while accessing and exiting the roof via an extension ladder that only extended 2.5 feet above the upper landing surface of the roof, on a low slope roof in Springfield, Illinois.       BLACKHAWK ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.1053(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1274715, CITATION NUMBER 1, ITEM NUMBER 1a, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 5, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 2126 S MACARTHUR BLVD, SPRINGFIELD, IL 62704.
