29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about November 6, 2019, Brandon Tedrick, as the controlling employer, had employees exposed to fall hazards in excess of 10 feet because each personal fall arrest system was not rigged in a manner to limit employee free fall to no more than 6 feet and prevent an employee from contacting a lower level, on a commercial construction site in Springfield, Illinois.
