29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection system, such as a personal fall arrest, travel restraint, or positioning system. a) Building 450, 2nd and McNair St., Fort Myer, VA 22211: On or about April 17, 2019, and times prior thereto, employees performed work at the loading dock platform when the bay door was open, with an unprotected edge of approximately 48" above the lower level, exposing employees to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.110(f)(6)(ii): Containers were not stored in a suitable enclosure or otherwise protected against tampering: a) Building 450, 2nd and McNair St., Fort Myer, VA 22211: On or about April 17, 2019, and times prior thereto, the employer stored approximately two portable LP gas cylinders on a platform at the loading dock outside the building, that were not in an enclosure and not protected against tampering, exposing employees to fire, struck-by or other hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Building 450, 2nd and McNair St., Fort Myer, VA 22211, warehouse: On or about April 17, 2019, and times prior thereto, the employer did not establish a program for energy control procedures, employee training and periodic inspection specific to the Wildeck Vertical Reciprocating Conveyor (merchandise elevator), where the employer had outside servicing personnel, including but not limited to Department of Public Works (DPW), perform service or maintenance, exposing affected and other employees to potential electrical, fall, or caught-in hazards. b) Building 450, 2nd and McNair St., Fort Myer, VA 22211, warehouse: On or about April 17, 2019, and times prior thereto, the employer did not establish a program for energy control procedures, employee training and periodic inspection specific to the Selco 3-phase, 460V vertical baler, where employees were allowed to clear jams that they could see using the Emergency shut off button, exposing employees to potential caught-in hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer posted a 300A for the Fort Belvoir Exchange with a different number of injuries/illnesses and employees than the establishment at Fort Myer. CORRECTED DURING INSPECTION.
