Employees were exposed to fall hazards while working at heights of approximately 12 foot above grade on a pump jack scaffold with deficiencies, including a lack of guardrails, improper overlapping of platforms, missing base plates and mud sills, and not properly braced at the top and bottom.

Where a top rail was installed, a mid-rail and bottom rail were not installed, exposing employees to fall hazards approximately 12 feet above grade.    b) Costal Club, 31611 Exeter Way, Lewes, DE - On or about February 27, 2019, the employer failed to ensure that guardrails were installed along all ends of platforms of a pump jack scaffold.  A guardrail was not installed on the end of a work platform, exposing employees to fall hazards approximately 12 feet above grade.

Coam Exterior, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451 (g)(1)(vii), which was contained in OSHA inspection number 10149901, citation number 1, item number 1a and was affirmed as a final order on April 21, 2015, with respect to a workplace located at Lot 134 Parkshore, Middletown, Delaware 19709.
