29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder: a) Lot 1027 Wright Way, North Shore Plantation Lakes, Millsboro, DE - On or about May 12, 2021, the employer failed to prevent employees from using an extension ladder to stand on a rung with just one foot and to reach out with both hands to access one of the vertical metal poles supporting the scaffold platform, exposing employees to a fall in excess of ten feet.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: a) Lot 1027 Wright Way, North Shore Plantation Lakes, Millsboro, DE - On or about May 12, 2021 employees were exposed to a fall hazard in excess of ten feet as the end of a platform greater than 10 feet in length extended over the scaffolding supports to lengths greater than 18 inches. Coam Exterior, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(5)(i), which was contained in OSHA inspection number 1381408, citation number 1, item number 1a, and was affirmed as a final order on August 27, 2019, with respect to a workplace located at Costal Club, 31611 Exeter Way, Lewes, DE.

Coam Exterior, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1381408, citation number 2, item number 1 and was affirmed as a final order on August 27, 2019, with respect to a workplace located at Costal Club, 31611 Exeter Way, Lewes, DE.
