29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Control of hazardous energy procedures (lockout/tagout) were not utilized on the roller machine. a) On or about February 24, 2019, machine operators did not utilize lockout/tagout procedures when changing plastic film and re-webbing the plastic film. b) On March 4, 2019, maintenance did not utilize lockout/tagout when servicing the roller machine.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about February 28, 2019, powered industrial trucks equipped with front-end attachments, other than factory installed attachments, were not marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, including but not limited, to the following: a) The Mitsubishi sit down rider LP forklift (truck ID E5) was equipped with an attachment but not marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. b) The Unicarriers side down rider electric forklift (truck ID Unicarriers) was equipped with an attachment but not marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. c) The Nissan side down rider electric forklift (truck ID D4) was equipped with an attachment but not marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.
