An uncapped oxygen cylinder was placed where it could be easily knocked down, exposing employees to fire and struck by hazards.

The interior of a spray booth which was cleaned once a year had accumulated 3-4-inches of combustible residues/deposits due to the spraying process, exposing an employee to a fire hazard.
