29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the jobsite, on or about February 28, 2019, employees used a Werner aluminum extension ladder that did not extend 3 feet above the upper landing surface to access and exit the second working level of a new residence during wood framing activities, thereby exposing employees to fall hazards of up to 11 feet onto concrete floor.
