OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee: (a) Oil States Energy Service Yard, Buckhannon, WV: On February 26, 2019 and times prior, employees were exposed to a hazard from holes in stay rod sleeves on a hydraulic setting tool that would allow an employee's finger to inadvertently enter during operation. Among other methods, one or more feasible and acceptable methods of abatement to correct this hazard would be: 1. Redesign the hydraulic setting tool mechanism to remove the hazard of the open holes 2. Ensure safety sliding cover is installed to protect employee's fingers from getting inside holes.
