29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 02/26/19 on the back right side of a house at 353 Porter Ave., Buffalo,NY :  Employees  used  an extension ladder to access and exit the high roof that was approx.  23 ft. above the ground. The siderails of that extension ladder only extended  approx. 18  inches  above the roof.       b) On or about 02/26/19 on the back left side of a house at 353 Porter Ave., Buffalo,NY :  Employees  used  an extension ladder to access and exit the pik on a ladder jack scaffold that was approx.  15 ft. above the ground.  The siderails of that extension ladder only extended approx. 11 inches above the pik.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 02/26/19, on the high roof on the back right side of a house at 353 Porter Ave., Buffalo,NY :  An employee engaged in roofing work on a steep roof was exposed to a fall of approx.  25 ft.  No fall protection system was being used .  Pitch of the roof was approx. 12 in 12.     b) On or about 02/26/19, on the lower gable roof on the back right side of a house at 353 Porter Ave., Buffalo,NY :  Two employees engaged in roofing work on a steep roof were exposed to a fall of approx.  17 ft.  No fall protection system was being used .  Pitch of the roof was approx. 12 in 12.     c) On or about 02/26/19, on the lower gable roof on the back left side of a house at 353 Porter Ave., Buffalo,NY :  Two employees engaged in roofing work on a steep roof were exposed to a fall of approx.  17 ft.  No fall protection system was being used .  Pitch of the roof was approx. 12 in 12.     NO ABATEMENT CERTIFICATION REQUIRED       EJC Consulting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1364270, citation number 1, item number 3, and was affirmed as a final order on 1/23/19, with respect to a workplace located at 33 Reservation Street, Buffalo, New York, 14207.
