29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another.  a) On or about 10/28/19, on a roofing job site, at a house at 26 Villa Ave., Buffalo, NY:  A ladder jack scaffold consisting of 3 support ladders and two 20 ft. long aluminum piks was erected on the west side of the house. The pik on the south end overlapped (bridged) the pik on the north end by about 12 inches at the middle support ladder. Employees go on the piks to install sheeting on the lower area of the roof.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about 10/28/19, on a roofing job site, at a house at 26 Villa Ave., Buffalo, NY:  An employee was standing on the flat surface of a dormer on the front of the house while taking measurements. The employee was exposed to a potential fall of approx. 9 ft. to the porch roof below because no fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about 10/28/19, on a roofing job site, at a house at 26 Villa Ave., Buffalo, NY:  Three employees were exposed to a potential fall of approx. 18 ft. from the aluminum piks of a  ladder jack scaffold erected on the west side of the house. The employees were installing sheeting on the lower area of the roof while standing on the piks of the scaffold, and no fall protection system was utilized.  NO ABATEMENT DOCUMENTATION REQUIRED    EJC Consulting dba The WNY Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1364270, citation number 1, item number 2, and was affirmed as a final order on 05/22/2019 with respect to a workplace located at 33 Reservation Street, Buffalo, NY 14207.
