29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about March 5, 2019, employees performing activities involving clean up of combustible dust, maintenance and repair of the equipment and their exhaust ventilation systems, were not provided with flame retardant clothing to protect against burns in the event of a fire and/or explosion related to combustible dust. b) On or about March 5, 2019, the employer did not ensure that employees wore aprons when working with nitric acid, sulfuric acid, and sodium hydroxide in the plating department.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee required to use Personal Protective Equipment (PPE) that covered when PPE is necessary, what PPE is necessary, how to don, doff, adjust and wear PPE, the limitations of PPE and the proper care, maintenance, useful life and disposal of the PPE: On or about March 5, 2019, the employees required to wear PPE have not been trained on when PPE is necessary, what PPE is necessary; how to don, doff, adjust and wear PPE; the limitations of PPE; and the proper care, maintenance, useful life and disposal of the PPE. Employees work with sodium hydroxide, sulfuric acid, nitric acid, combustible dust, metal working fluid, and lubricants. Employees are required to wear safety glasses, gloves, face shields and respirators when performing different job tasks.
