29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about March 5, 2019, the employer failed to ensure that machinery including, but not limited to the following, had machine guarding methods in place:    a) The Okuma LC20-2ST #1, S/N 6106-3503, did not have working interlocks.  b) The Okuma LC20-2ST #2, S/N 6212-3800, did not have working interlocks.  c) The Okuma LC20-2ST #3, S/N 6301-3740, did not have working interlocks.  d) The Okuma LC30-2SC #1, S/N 6108-1444, did not have working interlocks.  e) Morei Seiki SL-15 small lathe MFG#74  f) Threadmill machines 1A, 2A, 3A, 4A, 5A, 6A, and 121 were not equipped with guards around the hobs to prevent accidental contact.  g) The tool room Yamazaki Machinery Works Ltd. lathe was not equipped with an adequate chuck guard to prevent flying chips and sparks. Part of the guard was replaced with a piece of cardboard.  h) The Acme-Gridley Screw Machines, to include but not limited too, #17 did not have working interlocks.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    On and about March 5, 2019, multi CNC lathe employees were exposed to the following abrasive wheel bench grinders that had work rests that were not adjusted to 1/8 of an inch next to the abrasive wheels, to include but not limited too:    a) The Dewalt DW758 abrasive wheel bench grinder, located by Okuma LC-20-2ST #3, was missing the work rest and had openings of approximately 2 inches.    b) The Baldor abrasive wheel bench grinder was missing the work rest and had openings of approximately 2 inches.    c) The bench grinder (No information on grinder) located by the Haas CNC machine was missing the work rest and had openings of approximately 2 inches.
