29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    On or about March 7, 2019, the employer did not administer a continuing, effective hearing conservation program that included noise monitoring, training, audiometric testing and recordkeeping for employees engaged in wood pallet repair and manufacturing:    a) On March 7, 2019, employee #1 was exposed to continuous noise levels at 131.0 percent of the permissible daily exposure, or an equivalent sound level of approximately 91.9 dBA, during the 448-minute sampling period. Exposure calculations include a zero increment of 32 minutes not sampled.     b) On March 7, 2019, employee #2 was exposed to continuous noise levels at 227.9 percent of the permissible daily exposure, or an equivalent sound level of approximately 95.9 dBA, during the 447-minute sampling period. Exposure calculations include a zero increment of 33 minutes not sampled.    c) On March 7, 2019, employee #3 was exposed to continuous noise levels at 138.6 percent of the permissible daily exposure, or an equivalent sound level of approximately 92.4 dBA, during the 443-minute sampling period. Exposure calculations include a zero increment of 37 minute not sampled.    d) On March 7, 2019, employee #4 was exposed to continuous noise levels at 93.6 percent of the permissible daily exposure, or an equivalent sound level of approximately 89.5 dBA, during the 434-minute sampling period. Exposure calculations include a zero increment of 46 minutes not sampled.    e) On March 7, 2019, employee #5 was exposed to continuous noise levels at 94.9 percent of the permissible daily exposure, or an equivalent sound level of approximately 89.6 dBA, during the 261-minute sampling period. Exposure calculations include a zero increment of 219 minutes not sampled.    KIRK WOOD PRODUCTS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.95(C)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1142962, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 06/15/2016, WITH RESPECT TO A WORKPLACE LOCATED AT 10424 E 1400 NORTH RD, BLOOMINGTON, IL 61705.
