The ARMAC Thermoforming machine's stacker creates a pinch point hazard. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: JOB SITE: 925 Jeffco Executive Drive, Imperial, MO 63052, on or about 3/5/2019, live electrical parts were exposed due to a missing knockouts approximately one inch in diameter on the electrical light switch box on line 6, control panel on line and on the electrical outlet box on line 5. Employees were exposed to live parts in the machine shop area.
