29 CFR 1910.22(b): The employer did not ensure that each walking-working surface could support the maximum intended load for that surface: Shop Bay Mezzanine: Employees operated a forklift with a rated capacity of 6,000 pounds to load equipment pallets on the mezzanine. The mezzanine was not identified with the maximum intended load. Employees were exposed to struck-by hazards due to potential collapse of mezzanine.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Warehouse Storage: Employees used extension cords to energize hand-held electric tools placed on portable push carts, to perform Aircraft Launch and Arrest Device (ALAD) maintenance duties. The insulation on an extension cord was damaged. Employees were exposed to electrical shock hazards of 120 Volts.
