29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. NavAirWarCen Bldg 62: Self-contained eye wash stations had access blocked in locations where corrosives, acids, and solvents were used. Eye wash fluids had not been routinely changed (last date 2016). Employees are exposed to eye hazards and eye infections from chemicals and organisms such as Acanthameoeba, Pseudomonas, and Legionella.

29 CFR 1910.134(e)(7)(ii): Additional medical evaluations that complied with the requirements of 29 CFR 1910.134 were not provided when a PLHCP, supervisor, or the respirator program administrator informed the employer that an employee needed to be reevaluated. NavAirWarCen Bldg 62: Licensed Healthcare Professional (LHP) provided employer with medical re-evaluation requirements for employees using SCBA and air purifying respirators. The medical re-evaluations did not occur within the designated timeframe having a gap of 10 to 12 months. Employees were exposed to inhalation hazards from welding and hazardous chemical usage.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. NAVAIRWARCEN Bldg 62: Employees performed welding and parts cleaning operations with tight fighting air purifying respirators without fit tests.
