29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section. (a) An employee used a Makita Angle Grinder (model no. 9557NB, serial no. 2202485) without the wheel guard in place, exposing the employee to a struck-by hazard of flying debris from a shattered wheel.

(a) The employer did not provide and install adequate fall protection systems for employees conducting work on the second floor balcony. Employees were exposed to a fall hazard of 17 feet.
