(a) On or about March 31, 2021, on the fourth floor of the building directly below the roof access hatch, the employer did not insure that employees were protected from stepping thru the partially covered five (5) foot by eight (8) foot floor access hole. This exposed employees to a fall hazard of approximately 15 feet to 45 feet.
