29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about March 8, 2019, and at times prior thereto, the employer did not ensure machines were adequately guarded either at the point of operation or over the foot pedals that activate machines exposing employees to struck by and caught in hazards;  a)  The foot pedal that energized the Mubea Ironworker Machine was not guarded to prevent accidental actuation of the machine; and  b)  The Mubea Ironworker machine was not guarded effectively to prevent flying debris injuries.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.   On or about March 8, 2019, and at times prior thereto, the employer did not ensure the power cord that energized the foot pedal to the Mubea Ironworker machine was connected to devices such as a strain relief to prevent the internal wires from tension and the protective sheathing pulling away, exposing employees to electrical shock hazards.
