29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system.  Or, on roofs 50 feet or less in width, the use of a safety monitoring system alone:        On or about March 12, 2019, Skyline Roofing and Sheet Metal Company, Incorporated had employees exposed to fall hazards without fall protection on a commercial construction site in Springfield, Illinois.  These fall hazards included the following:      a. Large roof to small roof - in excess of 8 feet,   b. Small roof to parking lot - in excess of 13 feet,  c. Small roof to loading bay parking - in excess of 17 feet.    SKYLINE ROOFING AND SHEET METAL COMPANY, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1093519, CITATION NUMBER 1, ITEM NUMBER 1b AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 12, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 3450 SANDUSKY MALL BLVD., SANDUSKY, OHIO 44870.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      On or about March 12, 2019, Skyline Roofing and Sheet Metal Company, Incorporated had employees exposed to fall hazards, while accessing and exiting work levels with ladders that did not extend at least 3 feet above the upper landing surface of the roof, on a commercial construction site in Springfield, Illinois.   These fall hazards included the following:      a. Large roof, small roof - in excess of 8 feet and   b. Small roof, ground - in excess of 13 feet.    SKYLINE ROOFING AND SHEET METAL COMPANY, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1034566, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 11, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT THE CORNER OF COLLEGE AND VETERANS PKWY, NORMAL, ILLINOIS 61761.
