29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about April 17, 2020, three employees were conducting roofing operations without fall protection on an approximate 5 in 12 pitch roof that was approximately 9 feet from a lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladders' length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder were not secured at its top to a rigid support that will not deflect, and/or a grasping devices were not provided to assist employees in mounting and dismounting the ladders: On or about April 17, 2020, three employees used two portable ladders that were approximately 1.5 feet above an upper landing surface, and no grasping device was available to assist employees in mounting and dismounting the ladder.
