Storm Drain Inlet - On or about March 12, 2019 an extension ladder used to descend and access a storm drain inlet excavation did not extend at least 3 feet above the excavation landing. The employees were exposed to a fall hazard while descending and accessing the excavation. The employees were also exposed to a struck-by hazard due to approximately 4 foot of unsecured soil above the ladder.

The employees were exposed to a fall hazard due to an increased possibility of a foot slipping from the ladder rung.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. a. R & J Construction, Inc. - On or about March 12, 2019 a work crew was not trained on ladder usage and the recognition of ladder safety hazards. The work crew used an extension ladder to descend and access a storm drain inlet excavation. The employees were exposed to a fall hazard by not having the ladder setup correctly.
