29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about March 14, 2019, two workers were exposed to a scaffold collapse while working from a wooden job made box attached to the forks of a rough terrain fork truck. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about March 14, 2019, two workers were installing windows from a wooden job-made box, supported by the forks of a rough terrain fork truck. The employees were not protected from falls, exposing the employees to a fall of approximately 15 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
