29 CFR 1910.134(g)(1)(i)(B): Respirators with tight-fitting facepieces were worn by employees with a condition that interfered with the face-to-facepiece seal or valve function: Location - Facility On or about February 27, 2019, employees are exposed to respiratory hazards while wearing hoods over the head prior to donning half face air purifying respirators that effect the face to facepiece seal while working and spraying with paints containing chemicals such as, but not limited to acetone, methyl isobutyl ketone, titanium dioxide, and toluene.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: Location: Shop On or about February 27, 2019, employees were exposed to point of operation hazards while ripping wood without hood guards on the three Powermatic table saws and one Craftsman table saw.
