29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about March 12, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a. On or about March 12, 2018, at the above addressed jobsite, employees performing roofing operations on ladder jack scaffold approximately thirteen (13) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.  ECS Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(I) which was contained in OSHA inspection 1323574, citation number 1, item number 1 and was affirmed as a final order on September 21, 2018, with respect to a workplace located at 233 Meacham Avenue, Park Ridge, IL.

On or about March 12, 2019, at the above addressed jobsite, an employee performing residential roofing operations on a roof twenty (20) feet above the ground was not protected from falls, thereby exposing employee to fall hazards.  ECS Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1323574, citation number 1, item number 3 and was affirmed as a final order on September 21, 2018, with respect to a workplace located at 233 Meacham Avenue, Park Ridge, IL.
