29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:     a) On November 7, 2019, employees were exposed to eye injury while operating a pneumatic nail gun without eye protection during roofing work.   Josh Herion dba ECS Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 1385181, citation number 1, item number 1 and was affirmed as a final order on April 30, 2019, with respect to a workplace located at Lot 122 Tullamore Lane, Lemont, IL 60439.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     a)  On November 7, 2019, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 30 feet above the ground.  Josh Herion dba ECS Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1385181, citation number 2, item number 2 and was affirmed as a final order on April 30, 2019, with respect to a workplace located at Lot 122 Tullamore Lane, Lemont, Illinois 60439.  Josh Herion dba ECS Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1323574, citation number 1, item number 3 and was affirmed as a final order on September 21, 2018, with respect to a workplace located at 233 Meacham Ave., Park Ridge, Illinois 60068.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
