29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a)  At 12419 Lone Eagle, Lot 19 Osprey Ridge, Spanish Fort, Alabama:  On or about March 18, 2019 and at times prior; the employer allowed his employees to performing framing activities from a job made scaffold with a work platform that measured 6 inches wide exposing his employees to a fall hazard of 15.1 feet to the concrete floor below.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels are not protected by guardrail systems, safety net systems, or personal fall arrest systems, or an alternative fall protection plan which meets the requirements of 1926.502(k):  a)  The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on or about March 18, 2019, at 12419 Lone Eagle, Spanish Fort, Alabama, when employees performing framing activities from the top of framed walls without the use of conventional fall protection, are exposed to a fall hazard of up to 12.6 feet to the ground below.    b)  The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on or about March 18, 2019, at 12419 Lone Eagle, Spanish Fort, Alabama, when employees performing roof decking activities without the use of conventional fall protection, are exposed to a fall hazard of up to 14.6 feet to the ground below.

THE AMBROCIO LARA DBA AMBROCIO LARA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.102(A)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1321003, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 31, 2018 WITH RESPECT TO A WORKPLACE LOCATED AT 9819 COBHAM PARK DR., DAPHNE,  ALABAMA  IN OSHA INSPECTION NUMBER 1350979, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 24, 2019 WITH RESPECT TO A WORKPLACE LOCATED AT 23701 BLYTHEWOOD LANE DAPHNE, ALABAMA AND IN OSHA INSPECTION NUMBER 1330300, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 4, 2019 WITH RESPECT TO A WORKPLACE LOCATED AT 9565 CHAMBERWELL, DAPHNE, ALABAMA.
