29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about March 15, 2019, employees working near the back receiving area were exposed to burns and smoke inhalation and the employer did not ensure the portable fire extinguisher hanging near the back receiving door was readily accessible. The extinguisher was blocked by a plastic utility cart and the path to the extinguisher was approximately 18 inches wide.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: On or about March 15, 2019, employees working in the facility were exposed to burns and smoke inhalation and the employer did not ensure the portable fire extinguisher were inspected annually: a. The portable fire extinguisher hanging near the back receiving door was not inspected since 2016. b. The portable fire extinguisher hanging next to emergency exit 7 was not inspected since 2016. c. The portable fire extinguisher hanging outside the containment room next to the Eyesaline station and MSDS binder was not inspected since 2016.
