29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about January 15, 2025, employees working in the Receiving Area were exposed to electrical hazards because CubiScan Machine was plugged into an orange extension cord which was plugged into a purple relocatable power tap which was plugged into a metal receptacle outlet box. On or about January 15, 2025, employees working at the temporary Shipping Station were exposed to electrical hazards because the metal receptacle outlet box, containing knock-outs, was hanging unsecured and was not used in accordance with its intended purpose. On or about January 15, 2025, employees working at the temporary Shipping Station were exposed to electrical hazards because a relocatable power tap was secured to a pole by zip ties.
