29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch:    (a) Jobsite, Broad Street, Sewickley, PA:  On or about March 18, 2019 the platform on the working level of the scaffold erected inside the building was not fully decked or planked between the front uprights.  Employees were working on the scaffold chipping and grinding.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:    (a) Jobsite, Broad Street, Sewickley, PA:  On or about March 18, 2019 employees were working on the 2 towers of mason walk through scaffold that was 5 frames high (approximately 32 ft. plus) and 5 ft. wide and the scaffold had not been restrained to prevent tipping.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces shall not be used as a means of access:    (a) Jobsite, Broad Street, Sewickley, PA:  On or about March 18, 2019 there was no safe means of access to the uppermost working level of the scaffold erected on the outside of the building.  The employee accessed the scaffold from inside the building by climbing onto a stack of brick and then through a window opening onto the outriggers on the scaffold.  The employee was exposing to potential fall hazards of 25 plus feet to the ground.    (b) Jobsite, Broad Street, Sewickley, PA:  On or about March 18, 2019 there was no access to the working platforms on the scaffold erected inside the building.  Employees climbed the cross-bracing and frames to access the working levels.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) Jobsite, Broad Street, Sewickley, PA:  On or about March 18, 2019 an employee who had to climb through a window to access the outrigger platform work area was not protected from falling by personal fall arrest systems or adequate guardrails systems.  The fall hazard was approximately 26 feet to the ground below.    (b) Jobsite, Broad Street, Sewickley, PA:  On or before March 19, 2019 an employee who accessed the working level to provide materials such as block and mortar was not protected from falling by personal fall arrest systems or adequate guardrail systems.  The fall hazard was approximately 26 feet to the ground below.
