29 CFR 1926.454(c)(3): Where inadequacies in an affected employee's work involving scaffolds indicate that the employee has not retained the requisite proficiency. a) Jobsite, 429 Broad Street, Sewickley, PA: On or about April 8, 2019, an employee was working off of a scaffold platform to put a plate into a beam pocket, that was not fully decked or planked and was exposed to a potential fall of 33 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: a) Jobsite, 429 Broad Street, Sewickley, PA: On or about April 8, 2019, an employee used the cross braces and frames to come down to the ground from the scaffold because there was not a ladder to use for access/egress and he was exposed to a potential fall of approximately 33 feet to the ground.
