29 CFR 1926.760(a)(1):  Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  On 3/19/2019, employees of Barrera Construction, LLC were installing decking on a roof of a commercial building located at 7351 State Highway N, Dardenne Prairie, Missouri.  Employees were engaged in a steel erection activity on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level.  Employees were not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.

29 CFR 1926.761(b):  Fall hazard training.  The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section.  The employer did not institute a training program and ensure employee participation in the program.    On 3/19/2019, employees of Barrera Construction, LLC were installing decking on a roof of a commercial building located at 7351 State Highway N, Dardenne Prairie, Missouri.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed.  On 3/19/2019, employees of Barrera Construction, LLC were installing decking on a roof of a commercial building located at 7351 State Highway N, Dardenne Prairie, Missouri.  The employees were overloading the rated capacity of the extension ladder on the east side of the building, exposing employees to a fall to the ground below.
