29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      On or about March 14, 2019 and at times prior thereto, the employer did not ensure an energy control procedure was developed and implemented for employees to utilize while performing a maintenance task of unjamming material in the Flagler DH-48 Dimensional Line Roll Former machine.  The employees were exposed to caught-in and amputation hazards.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):        On or about March 14, 2019 and at times prior thereto, the employer did not develop clear and specific energy control procedures for each purpose and to address each specific steps in the procedure, and failed to enforce the utilization of the energy control procedure for each machine, including but not limited to, the Flagler DH-48 Dimensional Line Roll Former machine.  The employees were exposed to caught-in and amputation hazards.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.      On or about March 14, 2019 and at times prior thereto, the employer failed to certify periodic inspections were being performed by an authorized employee utilizing the energy control procedure(s).  The procedure(s) and annual reviews were not documented as required by 1910.147(c)(6)(I)(C) and (D) exposing the employees to being caught-in, struck-by and amputation hazards.

29 CFR 1910.147(c)(7)(iii):  The employer did not provide retraining in energy control procedures for employees who underwent a change in their job assignments, a change in machines, equipment or processes that would present new hazard(s) or a change in the energy control procedures:      On or about March 14, 2019 and at times prior thereto, the employer did not ensure all authorized employees performing lockout/tagout were provided training when there was a new machine or equipment (Flagler DH-48 Dimensional Line Roll Former) being operated, changes in the procedures, program, processes or machines (unjamming) that would create a new hazard; exposing the employees to being caught-in, struck-by and amputation hazards.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed.      On or about March 14, 2019 and at times prior thereto, the employer failed to ensure that the unused opening in the electric panel box powering the Wysong Press was effectively closed, exposing employees to electrical shock hazards.
