29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. On or about January 6, 2023 and at times prior thereto, where the employer did not clearly and specifically outline the steps for the placement locations of the lockout/tagout devices, and the removal and specific re-energization steps for each machine, including but not limited to, the Wysong Brake Press and the Roll Former machines. The employees were exposed to caught-in and amputation hazards.

The procedure(s) and annual reviews were not documented as required by 1910.147(c)(6)(i)(C) and (D), exposing the employees to being caught-in, struck-by, and amputation hazards. KPS Global was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(ii), which was contained in OSHA inspection number 1386679, citation number 1, item number 3 and was affirmed as a final order on )9/16/2019, with respect to a final abatement date of 09/24/2019, with respect to a workplace located at 4201 N. Beach St. FORT WORTH, TX 76137.
