29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about March 14, 2019, employees were exposed to amputation hazards when performing maintenance work on a Talley Machinery Super Sylon Flatwork 8-roll Ironer while the side access panel doors were not guarded to prevent employees from opening them and placing their hands and/or any other body parts in the danger zones between rotating gears and sprockets.
