On or about March 21, 2019, the employer did not ensure that employees who performed maintenance and/or service work on machinery and/or equipment such as the New Holland Skid Steer, model LS190, serial number N5M418227, exposing employees to struck-by and crush-by hazards. The manufacturer's lockout device was not engaged at the time of the incident.
