29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite: On or about January 9, 2019 and at times prior thereto; an employee was installing sheathing on truss framework without a fall protection system in place, exposing employee to falls from 6-8 feet to asphalt ground. b) Jobsite: On or about January 9, 2019, and at times prior thereto; Second and third floor balconies were missing guardrail systems which exposed workers to 10-20 foot falls during installation of siding. Note* Abatement certification IS required for this item.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: a) Jobsite: On or about January 9, 2019, and at times prior thereto; Second and third floor balcony guarding systems were found not fully secured to a support base, and there were instances where 2x4 boards were nailed together to create a top rail; exposing subcontractors to trips and falls from 10-20 feet.
