29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) Jobsite: On December 10, 2019 and at times prior thereto employees performed siding activities on the side of the building from an aerial lift.  The personal fall arrest system (harness) worn by one of the employees was not attached to a lanyard, exposing the employee to a 20 foot fall hazard to the ground below. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a) Jobsite: On December 10, 2019 and at times prior thereto employees performed siding activities above the third floor breezeway without a fall protection system in place.  The employees worked near an open sided floor on the backside of the building, exposing them to a 30 foot fall to the ground below.     b) Jobsite: On December 10, 2019 and at times prior thereto Consol, LLC removed a guardrail system while performing siding activities on the third floor breezeway.  The guardrail system was not replaced, exposing their employees and potentially employees of other sub-contractors to a 20 foot fall hazard to the ground below.    Consol, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR1926.501(b)(13), which was contained in OSHA inspection number 1387031, citation 1, item 1, and was affirmed as a final order on July 8, 2019, with respect to a workplace located at 303 West Penwood Street, Meridian, ID 83642.   Note) Certification of corrective action is needed for this item.
