29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.    a) Heritage Bag Company - Production Plant:  The employer did not ensure that authorized employees were trained on all aspects of the energy control program and all specific procedures for machinery including but not limited to:  Bag maker machine Battenfeld Gloucester model #35630-18, Serial #1/94 (Heritage # 6-0294), Heritage Machine #6-0294, Sam Tech Machine model #2012/S/N 95612, CDM Machine, Box maker machine model #TF600V, S/N TF600VB397.  Machine operators performed unjamming operations without using LO/TO procedures, on or about 3/14/2019 and 4/1/2019.

Authorized employees perform unjamming and maintenance operations on machinery including but not limited to:  Bag maker machine Battenfeld Gloucester model #35630-18, Serial #1/94 (Heritage # 6-0294), Heritage Machine #6-0294, Sam Tech Machine model #2012/S/N 95612, CDM Machine, Box maker machine model #TF600V, S/N TF600VB397, on or about 3/14/2019 and 4/1/2019.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Heritage Bag Company. - Production area:  The trash bag machine Model #35630-18, Model #1/94 did not have side guards to protect employees from ingoing nip points and rotating parts, occurred on or about 02/11/2019.    b) Heritage Bag Company. - box making area:  The Box maker machine Model #TF600V, S/N TF600VB397  had a guard that was insufficient to cover the ingoing nip points and rotating parts, occurred on or about 04/01/2019.

S/N 95612 had a damaged control in the reset button that was not covered as required, observed on 3/14/2019.    b)  Heritage Bag Company. - Manufacturing Plant:  The control panel  for the CMD machine has multiple openings on the seal bar temperature buttons exposing the internal wires, observed on 3/14/2019.
