29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the residential construction site located at 1307 Belindar in Raymore, Missouri: Employees were exposed to fall hazards. Framers were observed installing materials and working at height. Framers were exposed to a fall distance of greater than 22 feet eave to ground without use of fall protection. Tovar framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number1252312, citation number 1, item number 2 and was affirmed as a final order on 4/23/2018, with respect to a workplace located at Lot 318 NE 101st Street Kansas City MO.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the residential construction site located at 1307 Belindar in Raymore, Missouri: Employees were exposed to fall hazards. Framers were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing. Framers were exposed to a fall distance of greater than 12 feet eave to ground without use of fall protection. Tovar framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number1252312, citation number 1, item number 3 and was affirmed as a final order on 4/23/2018, with respect to a workplace located at Lot 318 NE 101st Street Kansas City MO.
